Your third-party references cannot live at the same address as you and at least one must be a family member and all must be people you speak with regularly. Personal information about your third party references is also collected. By completing this form, you acknowledge that you have obtained a consent for the collection of such personal information from your third-party references.

The following documents will be required to process your personal loan.

– Completed loan application form
– Two (2) Copies of your last pay stubs
– A statement showing the last 60 days of banking transactions
– Copies of two (2) pieces of ID – government issues with photo and signature
– Phone bill issued within 30 days to verify current address of applicant
– Void cheque or P.A.P. form

You certify that the information provided on this form is correct. You authorize verification of the information that you have provided, including use of a Consumer Credit Report for the purposes stated above and identified within our Privacy Policy. Fast Access Financial may contact any person or company in your application or any supporting documentation provided and you fully release all parties from liability for any damage that may result. You confirm that you have given your consent for Fast Access to verify the information that you have provided and to contact the references you have listed, for the purposes stated above and identified within our Privacy Policy.

By submitting an online application for a loan with Fast Access Financial, you agree that the information you have given to Fast Access Financial on this application is complete and accurate. You agree that you have read and consented to our Privacy Policy. You consent to, and accept this form as your written consent to authorize Fast Access Finance to contact your employer to verify your employment and to obtain a copy of your credit report from one of the Canadian credit reporting agencies.

Please Read Before Signing: As a condition of Fast Access Financial, lending me money based on the information provided above, I make the following representation: I am not contemplating filing for bankruptcy within the next four (4) months and have not consulted with an attorney or trustee concerning bankruptcy in the last two (2) months.

Please note that Fast Access Financial does not currently lend to or solicit business from U.S. residents or residents of provinces outside of Ontario, Canada. Please note that Fast Access Financial never requests a security deposit in exchange for processing a loan.
